Blepharitis — soothing your eyelids again

Blepharitis is a chronic inflammation of the eyelid margins. It is very common: about 25 to 30% of the general population, up to 47% after age 50, and even more among patients seen by an ophthalmologist. It causes eyelashes to stick together on waking, a burning sensation and redness along the lash line. It is rarely serious, but it is a fluctuating, recurrent condition that requires appropriate care. At the practice, I offer a comprehensive diagnosis to identify your type of blepharitis (anterior, meibomian gland dysfunction, Demodex or rosacea-associated) and a step-by-step treatment plan, from daily hygiene to innovative techniques such as IPL.

What is blepharitis?

Chronic inflammation of the eyelid margins: a fluctuating, recurrent condition, but one that can be very well controlled.

Definition and clinical significance

Blepharitis is a chronic inflammation of the eyelid margin (lash line). It is characterized by local signs (redness, telangiectasia, crusts, collarettes around the lashes) and symptoms (eyelashes stuck together on waking, burning, redness). Contrary to popular belief, it almost never leads to permanent lash loss, but it can significantly affect your quality of life.

It is a chronic, fluctuating condition — meaning symptoms vary with your hygiene routine, environment, stress levels and the seasons. It tends to come back if you stop treatment.

Prevalence and link with dry eye

Blepharitis is very common, particularly in patients with dry eye disease. Posterior blepharitis (MGD) is the leading cause of evaporative dry eye.

Why it matters: Inflammation of the eyelid margin worsens dry eye and increases the risk of chronic eye rubbing, which can lead to keratoconus. Early treatment prevents these complications.

At the practice, I distinguish two main forms based on anatomical location and underlying cause. Both affect the ocular surface as well as visual comfort.

Demodex — when harmless residents become a problem

Demodex folliculorum and brevis mites naturally live on the skin. But when they overpopulate, they cause a specific form of blepharitis.

The blepharitis mite

Demodex is a tiny commensal mite living in hair follicles and sebaceous glands. In most people, it causes no problems. But when its numbers rise (hence the term "demodicosis"), it triggers direct inflammation through enzymes and antigens.

Pathogenic Demodex infestation can be recognized by a characteristic sign visible at the slit lamp: cylindrical dandruff around the lashes ("collarettes") — white, translucent sleeves at the base of the lashes, highly suggestive of demodicosis.

Diagnosis

To confirm Demodex involvement, I remove an eyelash and look under the microscope for mites attached to its root.

Targeted treatment

Demodex is sensitive to essential oils and tea tree derivatives. I recommend:

  • Diluted 5-10% tea tree oil applied daily at home (Blephademodex or Cliradex wipes, based on purified terpinen-4-ol). Cleansing with 50% TTO is done only in the office under medical supervision because of the risk of irritation
  • Terpinen-4-ol (the active ingredient, 100% natural)
  • Topical ivermectin (Soolantra) if needed
  • Enhanced hygiene: cleansing 2-3× a day, no eye makeup during treatment

Treatment usually lasts 6-12 weeks. But be careful: stopping too early leads to a rapid relapse.

Systemic link

Blepharitis & rosacea (acne rosacea)

30-50% of patients with MGD (meibomian gland dysfunction) also have cutaneous rosacea.

The eye-skin connection

Rosacea is a chronic inflammatory skin condition of the face affecting the cheeks, nose, forehead and chin. Its skin signs are:

  • Erythema (persistent redness)
  • Telangiectasia (small dilated blood vessels)
  • Papules and pustules
  • Phymatous changes (thickening of the nose)

Eye involvement (ocular rosacea) is common and often more uncomfortable than the skin involvement. It presents as posterior blepharitis (MGD), dry eye and sensitivity.

My approach

At the practice, I manage the whole picture — eye involvement and the associated skin involvement — without systematically referring to a dermatologist. I start treatment according to severity:

  • Topical skin care: creams (metronidazole, ivermectin), sun protection
  • Oral doxycycline at an anti-inflammatory dose (50 to 100 mg/day for 6 to 12 weeks)
  • Daily eyelid hygiene, preservative-free tears, cyclosporine if needed
  • IPL for associated MGD

IPL is useful in ocular rosacea: it targets vascular inflammation (telangiectasia) and improves meibomian gland function (TFOS DEWS II). A dermatology opinion remains an option for severe or resistant skin disease.

Recognizing blepharitis

  • Stinging and itching along the eyelid margins
  • Burning and discomfort
  • Lashes feeling stuck together (especially on waking)
  • Paradoxical reflex watering
  • Gritty, sandy or foreign-body sensation
  • Fluctuating vision (better after blinking)
  • Sensitivity to light
When to suspect it — warning signs
  • Difficult mornings, eyes stuck together on waking
  • Fluctuating symptoms (good days and bad days)
  • Red eyelids, especially in the evening
  • Visible crusts at the base of the lashes
  • Lashes partially detached from the eyelid margin
  • Improvement with cleansing + warm compresses
  • Personal or family history of rosacea

How I make the diagnosis

1. Targeted history

I ask you specific questions: how long you have had symptoms, whether they fluctuate (better on some days?), your background (history of rosacea?), and eye-rubbing habits. This is crucial, because chronic eye rubbing can sustain or worsen blepharitis.

2. Slit-lamp examination

This is the cornerstone of the diagnosis. I examine in detail:

  • The eyelid margin: redness, telangiectasia, cylindrical collarettes (Demodex)
  • The lashes: stuck together, crusted, blepharospasm (forced blinking)
  • Meibomian gland expression: clear liquid secretion (normal) vs. paste-like/absent (dysfunction)
  • Signs of rosacea: facial redness, telangiectasia, papules
3. Infrared meibography

This imaging technique shows the condition of the meibomian glands: atrophy, shortening or disorganization are signs of MGD. It guides how intensive the treatment should be.

4. Tests for associated dry eye

I perform a tear break-up time (TBUT) test and a fluorescein test to assess epithelial damage. Dry eye is very frequently associated with posterior blepharitis.

5. Testing for Demodex

If I suspect demodicosis (visible cylindrical collarettes), I remove an eyelash and look under the microscope for mites attached to the root.

6. Skin examination

I look for signs of facial rosacea: redness of the cheeks, telangiectasia, papules. If present, I work together with a dermatologist.

Management

Step-by-step treatment

Step 1 — THE FOUNDATION
Daily eyelid hygiene

This is the cornerstone. To be done every day, for life:

  • Warm compresses: 10 minutes, 1-2 times a day. Use a heat mask warmed for 30 seconds in the microwave, or a Blephasteam device. Heat liquefies thickened secretions.
  • Eyelid massage: With a clean finger, massage the eyelid margin toward the lashes (10-20 strokes) to express the meibomian glands.
  • Cleansing: Ophthalmic eyelid cleanser (Blephasol, Blephagel, Naviblef, sterile wipes). At least twice a day.

In mild cases, daily eyelid hygiene may be enough on its own. In moderate to severe cases, it remains the essential foundation on which other treatments are added (azithromycin, doxycycline, IPL).

Step 2
Preservative-free artificial tears

Use tears with a lipid-based formula, preferably (Cationorm, Liposic, Refresh Optive Mega-3) rather than purely aqueous formulas. Lipid-based tears stabilize the tear film better.

Step 3
Short course of topical antibiotics (azithromycin)

Eye drops: Azydrop (azithromycin 15 mg/g) — 1 drop twice a day for 3 days, in line with the summary of product characteristics (SmPC). The course may be repeated depending on clinical response. Azithromycin has an anti-inflammatory effect beyond its antibiotic action.

Step 4
Oral antibiotics (doxycycline)

Doxycycline 50-100 mg once a day for 6-12 weeks. It works mainly through its anti-inflammatory effect (inhibition of collagenases and cytokines). Watch out for photosensitivity; it is contraindicated in pregnancy.

Step 5
Topical anti-inflammatory drugs

Short courses of corticosteroids (fluorometholone) for inflammatory flare-ups. Cyclosporine if severe dry eye is also present.

Step 6
Demodex-specific treatment

If demodicosis is confirmed:

  • Diluted 5-10% tea tree oil at home (Blephademodex, Cliradex, based on terpinen-4-ol) for 6-12 weeks; cleansing with 50% TTO is reserved for the office
  • Ivermectin cream (Soolantra) applied to the eyelids
  • Very strict hygiene: cleansing 3× a day, avoid makeup
Step 7
IPL (Intense Pulsed Light)

Indicated especially in MGD associated with rosacea. IPL targets vascular inflammation, improves meibomian gland function and reduces the Demodex load. 3 to 4 sessions, 1 to 4 weeks apart.

It shows how eyelid margin inflammation (blepharitis) triggers itching, which leads to rubbing, which in turn acts as an inflammatory amplifier. This vicious cycle applies to blepharitis, dry eye and keratoconus: treating eyelid margin inflammation early means preventing complications.

Long-term management

Monitoring & maintaining remission

It is a chronic condition

Unlike acute infections, blepharitis cannot be cured for good. It can be controlled, but it may come back if you stop treatment. This is normal and not a "failure" — it is the nature of the condition.

At the practice, we put together a tailored maintenance plan:

  • Daily hygiene to be continued indefinitely
  • Follow-up frequency (every 3-6 months)
  • Renewal of treatments if needed

Warning signs — when to see a doctor

You should come back promptly if you have:

  • A sudden worsening of symptoms despite good hygiene
  • Severe pain or new blurred vision
  • Signs of infection (pus, local warmth, eyelid swelling)
  • A white spot on the cornea or a corneal ulcer
  • Persistent symptoms despite following the treatment
